Community Telemedicine System is committed to protecting your personal information.

1. Personal Data Collected

Full name, date of birth, and gender
Contact details (email, phone, home address)
Health and medical records generated through the platform
Login credentials (passwords stored as encrypted hashes)

2. Purpose of Processing

To create and manage your patient account
To facilitate teleconsultation appointments
To send appointment confirmations and health notifications
To comply with legal and regulatory health requirements

3. Legal Basis

Processing is based on your consent (Sec. 12[a], RA 10173) and on the fulfillment of a telemedicine services contract.

4. Data Sharing

Your data will not be sold to third parties. It may be shared only with your attending physician(s) within the System, or with government health authorities as required by law.

5. Data Retention

Personal data is retained for a minimum of 10 years per Philippine medical records regulations.

6. Your Rights Under RA 10173

Right to be informed of processing activities
Right to access and obtain a copy of your personal data
Right to correct inaccurate or outdated information
Right to withdraw consent and request erasure (subject to legal limitations)
Right to lodge a complaint with the National Privacy Commission (NPC)

7. Security Measures

We implement HTTPS, hashed password storage, and role-based access controls.
